Following your application, you will be required to submit supporting documentation by mail or fax. Grant applications received online may be processed faster than offline applications.
Apply by Mail / Fax
To apply for a grant, please be sure to include the following information:
1. Name, address, telephone number, and contact person for your organization
2. Proposal summary (max 300 words)
3. Project description, including:
- Project goals
- Problems or needs the project addresses
- Who the project helps
- What groups are involved as collaborative partners
- Results you expect to achieve
- How the project relates to other efforts and organizations
- Plan for evaluating the success of the project
4. Background information on your organization:
- History and major accomplishments
- Current programs and activities
- List of board of directors and staff members
5. Organizational impact:
- Number of kids/adults in program
- Number of kids/adults who regularly attend program
- Number of parents directly involved in program
6. Budget information:
- Amount you are requesting from Rock for Reading
- Organization’s current annual operating budget
- Current project budget, if applicable
- Organization’s most recent fiscal year financial statement (expenses, revenue and balance sheet), audited if available
7. A copy of your IRS 501(c)(3) tax exempt verification letter. If you do not have 501(c)(3) status, you must include the IRS 501(c)(3) letter and authorization from the organization acting as your fiscal agent.
8. Supporting material – recent newsletter, annual report, articles, news clippings, letters of support, and other such materials.
